Smile Designers proudly participates with these insurance companies. However patients are responsible for any co-pays and/or deductibles that have not been met at the time services are rendered. If your maximum for the current calendar year is depleted; you are responsible for the full fee and/or balance of any fee over your annual maximum.

Payment is due at the time services are rendered. As a courtesy we will submit your family members' claims providing we have the most current and accurate information.

Third Party Provider: CARE CREDIT Cash, Credit Cards and Checks:

These are all acceptable forms of payment. If payment if full is not possible financial arrangements can be made through Care Credit.
